Terms & ConditionsThis section contains examples of information on Codes of Conduct and/or Terms & Conditions that are specific to Harwell & Hendred Youth FC. 

It is important that you take the time to familiarise yourself with the documents that are relevant to you, as you are deemed to have read, understood and agreed to the provisions contained in them when you 'sign' your membership application.

 

Alternative Payment Methods

We appreciate that not everyone can use, or is comfortable using the internet to make payment for goods and services. We are therefore delighted to accept cash and cheques. Please download, print off and complete the appropriate form below; these can be passed, with payment, to team captains, team managers, or other officers of the Club.

 

1. BEHAVIOUR/RESPECT

Harwell and Hendred Youth FC (HHYFC) fully support the FA’s Respect campaign and its values, which are incorporated within the Club’s Codes of Conduct. In parallel with completing this 2014/15 Membership Form, you (Parent and Player) will be asked to read and sign as having understood the Parent Code of Conduct and Player Code of Conduct Forms (attached to this document). Please help us to help your child by ensuring that we all set the right example regarding behaviour all times, including at training and especially on match day, and that you explain the correct behaviour to your child and why this is so important.

2. GLASSES

Please note that advice is that children should not wear normal glasses when playing or training. They need to wear sports goggles which can be prepared with prescription lenses, but which will prevent any injuring during play or training. Many High Street opticians sell sports goggles and you should contact them directly for advice.

3. PHOTOGRAPHY

The club may take team photos or action shots during the season for use on the club website, news items or publicity material, as may parents/club officials from opposition teams. If you do not wish your child to be photographed (e.g. they are under a protection order) or you may not want the pictures to be used in certain ways, e.g. on the web, please tick the appropriate box overleaf. Please be assured any information or discussions relayed to us on this subject will be dealt with appropriately and sensitively.

4. JEWELLERY

Please remind your child that all jewellery, metal/plastic hair bands and grips, wrist bands and watches must be removed before taking part in training or matches. Note that taping over ear studs, etc, is not acceptable. This is a mandatory FA rule and by ensuring that your child knows beforehand it will save everyone any possible embarrassment at training or on match day.

5. INJURY AND INSURANCE

The Club holds public liability including third party injury insurance as part of arrangements negotiated by our County Football Association (the Berks & Bucks FA). Parents are advised that they may wish to research their individual insurance requirements to ensure that they take out adequate insurance cover for any potential loss of earnings if they are required to looking after a child should they suffer an injury (e.g. a broken limb) whilst taking part in any football activity in connection with the Club. All due care and attention will be taken by members of the Club whilst all children are in their care and an accident book will be used to log any injuries, accidents or incident that requires treatment of any kind, and parents will be asked to sign the relevant log. Insurance certification and answers to any queries on this will be made available on request.

6. AGE REGULATIONS

Certain age groups have restrictions on the age a child must attain before he/she can play in that age group. Your relevant Manager/ Coach will be able to advise on these minimum and maximum ages. Please check when registering whether your child will be affected by any restrictions and feel free to discuss with us any concerns you may have.

7. LEAGUE REGISTRATION

This form is to register your child with HHYFC as a player. There will be further registration documents to be completed to register your child with the appropriate League (in 2014/15, these are the Oxford Mail Youth Football League or Oxford Mail Girls Football League), and you will be contacted at the appropriate time to complete these. For new players, new documents will require production of a copy of a birth certificate or passport identification as means of verification of age and you will also be asked to provide passport sized photo’s.
